请问鼻咽囊肿(tornwaldt cyst)是否一定要切除?
正好在健检中发现鼻咽有一出血点 相当紧张
做了EB IGA病毒检测 为阴性
再去看了耳鼻喉科 初断定为1cm鼻咽囊肿 (Tornwaldt cyst or Thornwaldt cyst) 医生说要在全身麻醉情况下(需住院两天)清除囊肿并送化验 网路上这方面资料较少
想请教有专业背景的网友:
1. 是否需要先照磁核共振 或电脑断层比较保险?
2. 其实那囊肿偶而会有分泌物 但对目前生活的影响不大
可否先暂时观察 看有无长大 而不动刀呢?

Tornwaldt's cyst
(Gustav Ludwig Thornwaldt, 1843-1910, German physician),(also called bursa pharyngea), a cystic notochordal remnant in the posterior nasopharyngeal wall, present in about 3% of the population. The cyst is located in the midline, between the prevertebral muscles. Most are asymptomatic, but they may occasionally cause nasal discharge, halitosis or other symptoms. They may become infected, causing prevertebral muscle spasm and a foul-smelling nasal discharge. A Tornwaldt's cyst appears as a hypodense cystic lesion on CT images; on MRI, the signal intensity is often rather high on T1-weighted images, presumably because of a high protein content . The differential diagnosis is a retention cyst in the nasopharyngeal mucosa.

治疗因为是良性所以简单，用(Curved pharyngeal scissors)剪开清除之，以造袋术或全切除为主，预后良好。不需要立即治疗。
